Our commitment

We use and share your health information to run our practice, improve your care, and support necessary business functions.

We may use or disclose your health information when required by law or permitted for purposes such as public health and safety.
Uses that require your written authorization
Most uses and disclosures of psychotherapy notes, uses for marketing, and any sale of your health information require your written authorization, which you may revoke at any time.

Our duties

If you believe your privacy rights have been violated, you may contact us using the details below, or file a complaint with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be penalized for filing a complaint.
